Position: Medical Case Manager – Full-Time (1 position available)
Salary: $40,000-$45,000 (Exempt)
Duties and Responsibilities:
Applicant must have an understanding of the targeted populations, communities and available resources and potential barriers to care. Responsible for maintaining confidentiality. Provide case management that links clients to medical care and other services in the community. Additional responsibilities include facilitating linkages, providing client advocacy and support, and supervising staff. Must document all aspects of work using the computer.
Qualifications:
A Masters level degree in Social Work and minimum of three years of full time professional case management in a public service agency. Two years experience working with persons with HIV/AIDS preferred. Demonstrated fluency in Spanish and English is preferred. Must have excellent written and oral communication skills. Must be proficient in the use of Microsoft Office, particularly Word and Excel. Applicant who has knowledge of relevant community resources preferred.

Position: Program Assistant – Part-Time (1 position available)
Salary: $13.00-$15.00 per Hour (based on experience)
Duties and Responsibilities:
This is program assistant position. The applicant hired will enter and manage data systems, monitor grant compliance, and assist program staff. The person hired will assist the Program Coordinator and Evaluation Assistant in monitoring grant compliance, entering and managing data when necessary, and completing internal and external reports for the Coordinator and team. The person hired will be responsible for maintaining the confidentiality of individuals served by Street Works.
Qualifications:
Applicant must possess a High School Diploma or GED, a minimum of three years of experience and must possess training, certification or education in office management. Experience in working with persons who are in active addiction or working towards recovery and/or are at risk of acquiring or transmitting HIV disease. Must possess good organizational skills. Expert computer knowledge and skills in using Microsoft Word and Excel, Adobe and data entry web applications. Prefer applicant with: 1.) an understanding and prior training in substance abuse and HIV/AIDS; 2.) experience in conducting research or evaluation interviews; and 3.) experience in managing research or evaluation data.
Also prefer individuals who possess the ability to work with diverse populations and have sensitivity to community norms, language, values, cultural beliefs, and traditions.

Position: Program Coordinator – Full-Time (1 position available)
Salary: $40,000- $45,000 (Exempt Position)
Duties and Responsibilities:
The applicant hired into the position must implement a newly funded outreach program and oversee the day-to-day operations. The person hired will supervise and evaluate all program staff and work closely with community partners and the Program’s external Evaluator. The duties will be performed primarily during traditional hours but because outreach is the primary focus, some non-traditional (evening and late night) hours will be required.
Qualifications:
Applicant must possess a Master’s in Social Work, Public Health or a related field and have 3 years of program management and supervisory experience. Must possess some knowledge of non-profit grants management. Prefer applicants with an understanding of substance abuse, HIV/AIDS and community resources and possess the ability to work with diverse populations. Applicants who are fluent in Spanish are encouraged to apply.
